SAMPLE COLLECTION

Sample Collected
by MyLAB Personnel
MyLAB personnel are available
to conduct sample collection
upon request. In such
condition, a certain amount
of fee will be charged
to the organization making
the request in the amount
mentioned in the price
list section of this catalogue.
The charge for sample
collection is separate
from the testing charges
billed to our customers.

Sample Collected
by Collection Agents
MyLAB also employs regional
sample collection agents
to assists in sample collection.
These
agents are assigned by
MyLAB in sample collection
only and are not qualified
to give technical advice.
Please make sure that
these agents comply with
all safety requirements
to prevent any unwanted
circumstances. These agents
are paid by MyLAB and
our customers are not
obliged to make any payments
directly to the agents.

Sample Collected
by Customers
The customers can also
collect samples themselves
if they wish to. This
can help them reduce their
cost in terms of not needing
MyLAB’s sampling
services. In such situation,
MyLAB’s technical
staffs will conduct training
sessions so that our customers
acquire the necessary
knowledge for them to
conduct sampling. An important
note: MyLAB can not be
held reliable in the event
that any damages occurs
to the sample before we
receive it if customers
wishes to conduct sampling
by themselves.

Test Request Forms
A completed and signed
request form must accompany
all samples. Please write
clearly to minimize documentation
error. It is very important
that all particulars are
provided as requested.
II. SAMPLE DELIVERY
II.I
Delivery by Hand
Farmers can deliver samples
collected to our laboratory
themselves. The samples
need to be packed in ice
and be kept away from
sunlight to prevent them
from being damaged. Please
make sure that request
forms accompany all samples
and all the samples are
properly labeled. This
is to ensure proper documentation
and processing of the
samples.

Delivery by Courier
Customers also have the
choice of sending their
samples using courier
service. In such cases,
please also ensure that
the samples are properly
packed in ice and kept
away from sunlight for
delivery. In addition,
please ensure that all
documentation accompanies
the samples.

Sample Reception
Hours
MyLAB’s normal hour
for receiving samples
is the same as its business
hours. It is advisable
that customers call the
laboratory upon sending
their samples to ensure
that our staffs are ready
to receive them. Samples
received after 5.00pm
will be kept in storage
for processing the next
day while samples received
after 5.00pm on the last
working day of the week
will be processed on the
first day of work the
week after.

DAMAGED SAMPLES
III.I
Samples Damaged
During Transportation
Any samples damaged prior
to reaching our facility
will not be of our responsibility
if they are collected
and delivered by our customers
themselves. We will only
take responsibility for
samples taken and delivered
by our sampling agents.
In either case, MyLAB
will inform customers
concerning the damaged
samples upon reception.

Samples Damaged In the
Laboratory
MyLAB will take responsibility
for samples that are damaged
in our facility due to
handling accidents. Customers
will be informed regarding
the damaged samples accordingly.
III.III
Billing Treatment for
Damaged Samples
Samples that have been
damaged either during
transportation or after
being received by the
laboratory will be deducted
from billing. Notes regarding
the type and amount of
damaged samples will be
placed together with MyLAB’s
result form given to our
customers. MyLAB will
also call our customer
notifying the damaged
samples.
IV.
LABORATORY TEST RESULTS
All test results will be
accompanied by respective
forms detailing each sample’s
ID. Customers will be informed
by mail and fax or phone
calls for urgent cases.
V.
PRIVACY IN REPORTING
All laboratory results are
treated with strictest confidentiality.
MyLAB will ensure that the
results are only available
to the respective customers.
VI.
CREDIT TERMS AND PAYMENTS
Credit terms are usually
30 days from the date of
the invoice. Checks should
be crossed and made out
according to the issuer
of the invoice.
VII.
QUALITY CONTROL
It is our utmost aim to
maintain the accuracy and
reliability of our laboratory
results. As such our laboratory
test are subjected to both
external and internal quality
audit (QA). MyLAB regularly
conducts cross checking
with VRI and other regional
DVS laboratory. As such
it is our sincere interest
to further enhance and establish
close collaboration with
other referral laboratories.
